Improved Vectors. Diseases and toxins inflicted by plague allies within 30 feet are harder to resist: targets suffer -1 to saves against them.

Sterile Contempt. The surgeon is immune to toxic damage and disease. Its augmetic surgical array counts as light armor (included in its Defense) and worse bedside manner.

Trepanning Injector. Melee Attack: +6 to hit, reach 5 ft., one target (Advantage against Grappled or Pinned targets). Hit: 1d8 + 2 piercing damage, and the target makes a Toughness Save DC 13 or takes 2d4 toxic damage as something experimental goes in.

Field Grafting. The surgeon stitches an adjacent downed plague ally back to function at half Vitality, or returns a fresh corpse to the fight as a shambling walker under its direction (once per corpse). The result is visibly wrong.

Aerosolize. The surgeon bursts a prepared sample over a 15-foot-radius zone within 30 feet. Creatures in the zone make a Toughness Save DC 13 or take 2d6 toxic damage, contracting a disease of the GM's choice on a critical failure.

Morale: Withdraws behind its patients when threatened and flees with its samples if the fight is lost — the work must continue.

Tactics: Keep it in the second rank undoing the party's kills — nothing enrages a table like a dead enemy standing back up. It spends actions on sustain first, offense last. The party must push through the patients to end the doctor, and the doctor has been improving the patients all fight.

Loot / Evidence / Hooks: Sample racks, grafting tools, and case notes that map everyone it has 'improved' and where each patient was sent.

The Plague Surgeon Heretek is a diseased butcher-medic who treats infection as a craft to be improved with tools. Its patients survive things they should not, in states they should not, and it is very proud of them.

Use this enemy to make plague forces regenerative and to horrify parties with battlefield surgery conducted mid-fight.

Moderate durability, weak direct combat, powerful ally sustain and undeath-adjacent recovery, disease weaponization, priority target that hides behind its work.
