'Urty Syringe. Melee Attack: +6 to hit, reach 5 ft., one target. Hit: 2d6 + 3 piercing damage, and the target makes a Toughness Save DC 13 or takes 1d6 toxic damage and moves at half speed until the end of its next turn. The contents are proprietary.
Get 'Im Back Up. One downed or bloodied allied ork within reach returns to the fight with 2d10 Vitality, visible new stitches, and one fewer original part. The procedure is loud.
Bone Saw Demonstration. A gratuitous flourish of the tools. Each non-ork creature within 30 feet that can see it makes a Willpower Save DC 13 or is Frightened until the end of its next turn.
Morale: Retreats with the mob rather than breaking; alone, it surrenders cheerfully and immediately offers its services, which is worse.
Tactics: Keep the painboy behind the front rank recycling casualties, and make the party watch orks they killed stand back up. Its aura plus its surgery can double a mob's effective endurance; screen it with patients who owe it money.
Loot / Evidence / Hooks: Its tool harness holds serums, saws, and syringes of unregistered xenos pharmacology — priceless to a biologis magos, hazardous to everyone in the same room.

GM ReferenceThe painboy is ork battlefield medicine: a surgeon-enthusiast with a syringe the size of a fence post and a genuine gift for keeping orks in the fight. Its patients survive at rates that offend Imperial medicae science, and scream accordingly.
Use this enemy to make an ork mob regenerate mid-fight, and to give the party a support target they will grow to hate.
Threat Profile: Medium durability, mob-sustaining battlefield surgery, horrifying melee syringe, support target of the highest priority, orks fear it more than the party.
