Alchemicus Familiar. Its serum-bearing familiar scurries between allies: once per round, the Biophagus may apply a serum effect (including Battlefield Augmentation) to a cult ally within 30 feet through it.

Grotesque Expertise. Aberrants and metamorphs within 30 feet gain +1 damage on melee attacks while the Biophagus observes and adjusts them.

// Actions

Injector Goad. Melee Attack: +5 to hit, reach 5 ft., one target. Hit: 1d6 + 2 piercing damage, and the target makes a Toughness Save DC 13 or suffers a random chemical effect: spasms (Stunned until the end of its next turn), slowing (Movement halved for one round), or searing pain (1d6 toxic damage and Disadvantage on its next attack).

Battlefield Augmentation. The Biophagus injects a cult ally within reach or via its familiar: that ally gains +2 damage on its attacks and 10 temporary Vitality for the rest of the scene, at a cost the GM narrates later.

Shatter the Vats. The Biophagus ruptures lab equipment within 30 feet, creating a 10-foot toxic cloud that lasts until the end of the scene. Non-cult creatures that enter or start their turn in the cloud make a Toughness Save DC 13 or take 2d6 toxic damage and are Blinded until the start of their next turn.

Morale: Retreats to protect its research; it abandons fighters without hesitation but dies badly defending the brood-nest incubators.

Tactics: Keep it adjacent to the biggest monster in the room, actively making it worse each round the party leaves it alone. Its lab terrain — vats, feed lines, specimen cages — should be both hazard and objective. Killing it early is the correct play; make the path to it cost something.

Loot / Evidence / Hooks: Serum racks, gene-splicing notes, and a specimen ledger tracking each aberrant by donor name — evidence, cure research, or heresy, depending on whose hands it reaches.
